Scope
This Consumer Health Data Privacy Policy supplements our Privacy Policy. It describes how Premier Sperm Donors, LLC d/b/a Premier Sperm Bank ("Premier," "we," "us," or "our") collects, uses, shares, and protects consumer health data related to fertility and reproductive services, including where laws such as Washington's My Health My Data Act apply.
We do not sell consumer health data. We do not share consumer health data for cross-context behavioral advertising.
Consumer Health Data We Collect
Depending on how you use our services, consumer health data may include:
- Fertility, treatment, insemination, clinic, recipient, and family-building information you provide during account setup, checkout, consultations, support, or signed authorizations.
- Donor-selection activity, favorites, cart activity, order records, shipping or storage choices, clinic delivery details, and related service records when they identify or can reasonably be associated with fertility or reproductive services.
- Verification or compliance information you provide for service, security, eligibility, consent, or legal purposes.
- Phone, text, email, and other communications with us about fertility, reproductive services, donor sperm, shipping, storage, treatment coordination, or related support.
- Online activity associated with our fertility-related services, such as page views, checkout-step progress, and product-usage events, where collected with consent and configured to minimize sensitive content.
Sources
We collect consumer health data from:
- You directly when you create an account, place an order, complete checkout, submit forms, sign authorizations, contact us, or communicate with our team.
- Your browser or device when you use our website, subject to your privacy choices and our consent controls.
- Service providers and operational systems that support authentication, payments, fulfillment, customer relationship management, customer support, phone, text and email communications, analytics, security, and compliance.
- Clinics, healthcare providers, shipping recipients, or other contacts when you direct us to coordinate service with them.
Purposes for Collection and Use
We collect and use consumer health data to:
- Provide fertility and donor sperm services you request.
- Create and manage accounts, carts, favorites, orders, signed authorizations, shipping, storage, clinic delivery, and customer support workflows.
- Communicate with you by phone, text, email, or other channels about services, orders, account activity, consent documents, administrative notices, and support requests.
- Coordinate with clinics, providers, payment processors, shipping carriers, and other service providers as needed to provide requested services.
- Maintain legal, regulatory, accounting, donor-family-limit, consent, compliance, and dispute-resolution records.
- Protect the security and integrity of our services; detect, prevent, and investigate fraud, abuse, or security events.
- Improve website functionality and checkout usability using consented product analytics configured to minimize sensitive content.
- Comply with law, legal process, and regulatory obligations.
Sharing Consumer Health Data
We disclose consumer health data only when needed to provide requested services, at your direction, with your consent, with service providers that process data for us, or when permitted or required by law.
Categories of consumer health data disclosed may include account, order, clinic, shipping, support, consent, technical, analytics, and compliance records related to requested fertility services.

- Payment, financing, shipping, and fulfillment providers.

We do not sell consumer health data, share it for advertising, or share it with corporate affiliates.
Your Rights
Depending on applicable law, you may have the right to confirm whether we collect, share, or sell consumer health data about you; access that data; receive a list of third parties or affiliates with whom we have shared or sold consumer health data; withdraw consent for future collection or sharing; and request deletion of consumer health data.

You do not need to create a new account to exercise these rights, but we may ask you to use an existing account or provide information reasonably necessary to authenticate the request.

We will respond to authenticated requests within the timelines required by applicable law. If we deny a request, you may appeal by replying to our decision notice or emailing care@premierspermbank.com with the subject line "Consumer Health Data Appeal."
